Title: LMSW – Home Health
Location: Portland, OR
Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 – 16:30)

Job Responsibilities and Required/Preferred Skills & Requirements:

Job Responsibilities:

• Provide home health social work services to adult and geriatric patients

• Conduct comprehensive biopsychosocial assessments and initial evaluations

• Develop, implement, and revise individualized plans of care and goals

• Coordinate care with interdisciplinary teams and community resources

• Support discharge planning and transitions of care

• Educate patients and caregivers to facilitate safe discharge and continuity of care

• Document assessments, care delivery, education, and patient response per plan of care

• Participate in care conferences and collaborative care activities

• Address patient safety, vulnerability, and high-risk interventions as needed

Required Qualifications:

• Licensed Master Social Worker (LMSW), state licensed

• Minimum of 2 years of social work experience

• At least 1 year of skilled home health experience

• BLS certification

• OASIS experience required

• Proficiency with electronic documentation systems (Epic preferred)

• Must hold both Oregon and Washington social work licenses

• Reliable personal transportation required

Preferred Skills/Experience:

• Knowledge of adult protective services, guardianship, and power of attorney

• Experience with advance directives, ADA regulations, and high-risk patient populations

• Familiarity with community-based service programs, including Agency on Aging, HCBS, Medicare waivers, and Veterans programs

• Strong care coordination, case management, and utilization review skills

• Experience in home healthcare and care coordination settings
