Description

Job Title / Discipline: Therapy / Rehabilitation – Physical Therapist
Specialty: PT – Home Health (Adult)
City: Colchester
State: VT
Shift: Days – 5x8s (08:00 – 16:30)
Hours per week: 40
Duration: 20 weeks (may extend up to 32 weeks)

Requirements:

Licensed Physical Therapist – REQUIRED
Minimum 3 years recent Home Health experience – REQUIRED
Epic EMR experience – REQUIRED
Prior travel experience required (2 travel assignments preferred)
Strong clinical & assessment skills
Comfortable seeing 5–6 patients per day
Fluent navigating multiple EMRs
Reliable personal transportation REQUIRED (no public transit access)
