Description

Title: Physical Therapist – Home Health
Location: Portland, OR (East Portland and surrounding areas)
Duration: 13 weeks
Shift: Days, 5x8s (08:00 – 16:30)

Job Responsibilities and All Required and Preferred Skills and Requirements

Role Overview

Provide skilled physical therapy services to adult and geriatric patients in the home health setting.
Manage a caseload of approximately 5–6 patients per day (about 30 productivity points).
Perform comprehensive evaluations, develop and update plans of care, reassess progress, and complete discharge planning.
Document accurately using Epic Remote Client, Rover, and OASIS.
Educate patients and families on home exercise programs, safety, and injury prevention.
Travel independently between patient homes; visits are generally grouped to minimize drive time.

Required Qualifications

Experience: Minimum 1 year of skilled home health physical therapy experience.
Certifications: BLS.
Documentation: OASIS experience required.
Transportation: Must have reliable personal transportation.
Patient Population: Adults and geriatrics.

Preferred Qualifications

Epic charting experience.
TDN certification.
Experience across acute care, LTAC, outpatient, rehab hospital, and SNF settings.
