Title: Occupational Therapist (OT) – Skilled Nursing Facility / Outpatient
Location: Troy, NC
Duration: 13 weeks
Shift: Day shift, 5x8-hour shifts (primarily 8:30 AM – 5:00 PM; one day/week 8:00 AM – 4:30 PM)

Job Responsibilities:

• Provide one-on-one occupational therapy services across multiple settings including SNF, acute swing bed, outpatient, and acute care.

• Majority of assignment involves SNF and acute swing bed coverage with occasional outpatient care.

• Provide outpatient and acute split coverage one day per week, including orthopedic, gait/balance, post-surgical, aquatic therapy, and neurological patients.

• Manage patient caseloads:

  • SNF/acute: approximately 6–10 patients per 8-hour shift.

  • Outpatient/acute: approximately 8–12 patients per 8-hour shift.

• Deliver individualized treatment plans focused on functional improvement and patient safety.

• Collaborate with interdisciplinary healthcare teams as needed.

• Complete required documentation in a timely and accurate manner.

Required and Preferred Skills & Qualifications:

• Occupational Therapist eligible to work in SNF, acute, and outpatient settings.

• Pending license accepted at submission.

• Ability to manage diverse patient populations and diagnoses.

• Experience with orthopedic, neuro, gait/balance, and post-surgical rehabilitation preferred.

• Strong patient communication and time management skills.

• Ability to work independently with one-on-one patient care.

• Travel experience not required.
