Job Summary
The Integrated Case Manager is responsible for coordinating care and discharge planning for acute inpatient populations. This role requires strong clinical judgment, social work expertise, and the ability to work directly on the patient floor. The manager is specifically seeking candidates with a social work background (MSW/LMSW) who can quickly adapt to a fast-paced acute care environment.
Required Experience
-
Minimum 5 years of acute inpatient case management experience

Strong experience in acute discharge planning

Previous experience using CarePort

Ability and willingness to work directly with patients on the unit

Must be able to acclimate quickly and work independently
Required Education (One of the following)
-
Nursing Degree
OR -
Master of Social Work (MSW)
Required Certifications & Licensure
-
Registered Nurse (RN) with a valid, unrestricted Michigan license
OR -
Licensed Master Social Worker (LMSW) with a valid, unrestricted Michigan license

BLS Certification (AHA required)
⚠️ Manager Requirement:
The manager is only reviewing MLSW/LMSW-certified candidates due to the need for a strong social work background.
Preferred Qualifications

Previous charting experience with AllScripts

Experience with GEMS/Sunrise systems
