Description

Job Title: Hospice Registered Nurse (RN)

Day 5x8-Hour (08:00 - 17:00)

Position Summary:
This center is needing a Hospice RN with Homecare Homebase (HCHB) experience who can hit the ground running. The RN will be assigned a tablet, see patients the same day each week, and participate in the on-call rotation.

Work Schedule:

On-call rotation
Monday – Friday (8:00 AM – 5:00 PM)

Position / Specialty: RN – Hospice

Charting Requirements:

Homecare Homebase (HCHB) & OASIS
Must complete charting while at patient home

Average Patient Load: 4–6 patients per day

Points / Hours:

On average 30 points per week (equivalent to ~40 hours per week)

Guaranteed Hours: Varies from facility to facility

Types of Cases:

In-home patient care
Interdisciplinary care coordination
Plan of Care development and oversight
Geriatric population

EMR: Homecare Homebase (HCHB)

Experience Required:

Minimum 1 year Home Health experience
2 years' experience overall as a clinician
