Description

Care Manager - ED
Location: 1400 Pelham Parkway South, Bronx New York 10461
Facility: Jacobi Medical Center
Shift: 10:00 AM-6:00 PM

Requirements:

5 years minimum of clinical experience as a Registered Nurse.
Experience with Care Coordination, Utilization review and discharge planning computer literacy.
Associate's Degree (ASN, ADN) from an accredited Nursing Program required.
RN Licensed as a Registered Nurse in the State of New York required.
PRI and Screen certification required.
BLS required.
Weekends/ Holidays required: No
