Required Experience
• Minimum 4 years of clinical nursing experience
• Strong interpersonal and communication skills
• Knowledge of infection control and patient safety
• Proficiency with wound/ostomy technology and equipment
• Ability to travel locally
• Innovative, patient-focused approach
Preferred Experience
• 2 years of wound and ostomy care experience
Education
Required: Bachelor of Science in Nursing (BSN)
Licensure & Certifications
Required:
• Active California RN License (good standing)
• CWON / Wound & Ostomy Certification (Required)
Additional Notes
• Local travel required (must be licensed & insured driver)
• Must maintain CWON certification
• Professional clinical educator role within home health system
Position Summary
The Wound Ostomy Nurse serves as a clinical expert for ostomy, continence disorders, acute and chronic wounds, skin breakdown, and fistula management. This role provides direct patient care, staff education, and supports hospital-wide initiatives to prevent hospital-acquired pressure injuries.
Key Responsibilities
• Provide direct wound and ostomy care (inpatient & outpatient)
• Educate patients and families pre- and post-operatively
• Conduct bedside teaching and nursing staff in-services
• Assess all pressure injuries Stage 3 and above
• Manage and report hospital-acquired pressure injuries (HAPI)
• Support hospital initiatives to reduce HAPIs
• Evaluate skin and wound care products and equipment
• Stay current on wound care best practices and prevention measures
