Job Title: Licensed Master Social Worker (LMSW) – Home Health

Location: Tukwila, WA 98168
Shift: Day Shift | 5x8-Hour (08:00 AM – 04:30 PM)

Position Overview

We are seeking an experienced Licensed Master Social Worker (LMSW) for a Home Health setting in Tukwila, WA. The ideal candidate will have recent home health or geriatric experience, strong communication skills, and the ability to manage patient care across a flexible territory.

Key Responsibilities

• Provide comprehensive psychosocial assessments and interventions for geriatric patients in home health settings.

• Develop and implement individualized care plans in collaboration with interdisciplinary teams.

• Ensure timely and accurate documentation in the EMR system (HCHB).

• Communicate effectively with patients, families, and healthcare professionals.

• Manage caseloads efficiently, averaging 5–7 patients per day (to be confirmed during interview).

• Maintain flexibility in covering assigned territory (up to a 60-mile radius).

Required Qualifications

• Active LMSW license (license must be in hand).

• Minimum 2 years of recent LMSW experience (home health preferred).

• BLS certification required.

• Experience with geriatric patient populations.

• Strong documentation and communication skills.

• Ability to work independently in a home health environment.
