Description
Job Title: Licensed Master Social Worker (LMSW)
Location: New York, NY 10016
Assignment Length: 13 Weeks
Department: Social Work (Non-Behavioral Health)
Schedule: Monday–Friday, 9:00 AM–5:00 PM (35 hours per week, 1-hour lunch)
Position Overview
We are seeking a Licensed Master Social Worker (LMSW) with hospital-based experience to support patients in an inpatient medical setting. The ideal candidate will have experience working with medical/surgical and rehabilitation populations and be comfortable collaborating within a multidisciplinary care team.
Required Qualifications

Minimum 1 year of experience as a Licensed Master Social Worker

Experience in inpatient medical, surgical (med/surg), or rehabilitation settings preferred

Master's Degree in Social Work from an accredited program

Active LMSW license issued by the New York State Education Department (NYSED) (Primary Source Verification required)
Key Responsibilities

Provide psychosocial assessments and care planning for hospitalized patients

Support discharge planning and coordinate post-acute services

Collaborate with physicians, nurses, and interdisciplinary team members

Offer crisis intervention, counseling, and patient/family support

Connect patients with appropriate community resources and services

Maintain accurate and timely documentation in accordance with hospital policies
