Description

Title : Physical Therapist
Location : Concord, CA 94520
Duration : 13 weeks
Shift : Day 5x8-Hour (08:00 - 17:00)
Specialty: Home Health
Coverage Area: Up to a 60-mile radius from the hub/office

Experience Requirements:

Minimum 1 year of recent PT experience required
OASIS experience preferred
HCHB (Homecare Homebase) EMR experience required

Workload & Schedule:

Average 5–7 patients per day
40 hours guaranteed
On call required every 3rd to 4th weekend

Patient Population:

Geriatric cases

Eligibility:

First-time travelers accepted
Local candidates accepted at travel rate
Active license in hand required

Certifications:

BLS required
