Description

Title : Physical Therapist – Outpatient
Location : Florence, OR 97439
Duration : 13 weeks
Shift : Day 4x10-Hour (07:00 - 17:00)

Schedule Flexibility: Block scheduling available (Monday–Thursday or Tuesday–Friday)

Experience Requirements:

Minimum 1 year of experience preferred
Epic EMR experience preferred

Licensure & Certifications:

Active Oregon Physical Therapy License (required)
BLS certification (required)
Pending state licenses not accepted

Traveler Details:

First-time travelers accepted
Local candidates not accepted

Weekend / On-Call Requirements:

Monday–Friday schedule preferred
No weekends required
No on-call responsibilities

Floating Requirements:

No floating within scope required
No floating to sister facilities
May assist as an Admission Coordinator or Rehab PPS Coordinator as needed

Patient Volume & Productivity:

Average caseload: 9–11 patients per day (based on schedule and census)
Productivity expectations:
- &Client;0.45 for Outpatient
- &Client;0.60 when splitting between Inpatient/Outpatient

Clinical Focus / Top Procedures:

General orthopedic care
Total joint rehabilitation
Acute and chronic pain management
Balance deficits
General neurological conditions
General medical cases
Upper extremity injuries
Splinting (required skill)

Patient Population / Insurance:

All payer sources
Primarily Medicare and Managed Medicare
