Description
Title: Physical Therapist – Inpatient Rehab
Location: Santa Rosa, CA
Duration: 13 weeks
Shift: Day, 5x8 (08:30 – 17:00), schedule could be Tuesday–Saturday or Sunday–Thursday
Job Responsibilities and Required/Preferred Skills:

Provide inpatient rehab therapy for adult and geriatric patients in an acute care rehabilitation hospital (daily census ~185).

Treat patients with cardiopulmonary, neurologic, orthopedic, oncologic, and geriatric conditions.

Deliver specialized care for: post-stroke rehabilitation, orthopedic injuries, joint replacements (total hip/knee), prosthetic training, cognitive disorders, neurological conditions, lymphedema management, and cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD, cystic fibrosis).

Perform patient evaluations, assessments, documentation, plan of care, reassessments, short- and long-term goals, and discharge planning.

Use modalities such as cryotherapy and moist heat.

Manage equipment and devices including Hoyer lifts, sit-to-stand lifts, gait belts, crutches, walkers, specialty beds, prosthetics, shoulder slings, and supplemental oxygen.

Collaborate with interdisciplinary teams: physical therapy, respiratory services, social services, pharmacy, radiology, rapid response teams, phlebotomy, and interpretation services.

Monitor vital signs, assess pain, strength, arousal, attention, sensory response, and skin integrity.

Educate patients and families on home exercise programs, safety, medications, and injury prevention.

Perform safe patient transfers, lifting, mobility training, and adaptive device usage.

Maintain documentation and charting in Epic EMR.

On-call coverage required; must float between the primary facility and nearby sister hospitals as needed.

Required Experience: Minimum 1 year in acute care; inpatient rehab or Level II trauma experience preferred.
Required Certification: BLS; other specialty certifications preferred. -
Provide at least 2 supervisory references covering the past 1 year of employment.

Must have a valid CA license; pending license not accepted.

Willingness to work assigned schedule including Saturdays and major holidays as required.
