This center is needing a Hospice RN with HCHB experience and can hit the ground running. - 30 points per week = 40hrs per week. They will need to be assigned a tablet and see patients same day week and will need to be in the on-call rotation as well

Average # of patients/day: 4-6 patients per day
Points per day/week: How are points broken down? On average 30 points per week

Types of Cases: In-home Patient Care, Interdisciplinary Care Coordination, Plan of Care Development and Oversight, Geriatric
Population, Homecare Homebase EMR (HCHB)
Years of experience REQ: Min 1 year Home Health, 2 years’ experience overall as a clinician.
First-timers accepted: (Y/N) Yes.
Weekend REQ: Weekends as needed, On call
Certs REQ: BLS and valid License in hand.
Is on-call REQ? (Y/N) Must be open to 1 night of call during the week, discuss upon interview
Territory/County covering: Must be willing to travel up to 50 miles outside home base
Mileage reimbursed? IRS Standard
Locals accepted: (Y/N) What is the local radius rule? Yes, 50-mile radius for locals.
Pending License accepted: (Y/N) No, must have valid license in hand at time of sub.

Job Description: The Registered Nurse is responsible for administering quality, skilled nursing care to patients from diverse populations, in a variety of home environments. The RN assumes responsibility for coordinating care with the patient and family, certifying the provider, and all members of the interdisciplinary team. The RN performs comprehensive assessments of patients and collaborates with the certifying provider to develop a Plan of Care that meets the needs of the patient.
