Job Title: Registered Nurse Case Manager (Inpatient/acute)
Location: Detroit, MI
Duration: 13 Weeks
Shift: 8 am-4:30 pm; weekend rotation

Job Summary:

Seeking an experienced Integrated Case Manager to coordinate and manage care for patients with complex medical and social needs. This role focuses on assessment, care planning, discharge coordination, and improving patient outcomes across the care continuum.

Key Responsibilities:

• Conduct comprehensive patient assessments (medical, psychosocial, functional)
• Develop and implement individualized care plans
• Coordinate discharge planning and transitions of care
• Collaborate with interdisciplinary teams for complex case management
• Provide patient/family education, advocacy, and resource coordination
• Monitor patient progress and adjust care plans as needed
• Support quality improvement and risk reduction initiatives

Requirements:

• 5 years Case Management experience
• 2 years recent Acute Inpatient Case Management (REQUIRED)
• BSN or MSW required
• Active MI RN License or LMSW
• Strong discharge planning experience
• Must adapt quickly (limited training provided)
