Description

Position: Travel RN – Home Health
Location: Bastrop, TX
Shift: Nights | 5x8
Duration: 13 Weeks (Travel/Local Accepted)

Job Summary:
We are seeking a skilled and compassionate Registered Nurse (RN) for a Home Health travel assignment in Bastrop, TX. The RN will provide high-quality, patient-centered care in home settings, ensuring comfort, safety, and improved patient outcomes.

Responsibilities:

Deliver skilled nursing care in patients' homes
Perform comprehensive assessments and develop individualized care plans
Complete accurate, real-time documentation using HCHB & OASIS
Manage 4–6 patients per day
Collaborate with interdisciplinary care teams
Educate patients and families on care plans and health management

Requirements:

Active RN License (required)
BLS Certification
Minimum 2 years clinical experience (1 year Home Health preferred)
Proficiency in HCHB & OASIS
Willingness to travel and participate in on-call rotation
