Description
Job Title: PT Home Health
Location: San Jose, CA 95134
Start Date: 05/04/2026
Duration: 26 weeks
Shift: Days, 5x8 (08:00–17:00)
Requirements:
• 1 year Home Health experience (2 years overall)
• Minimum 23–24 visits/week
• Experience with geriatric & wound care patients
• Kinnser/Wellsky preferred
• Valid state license required
Certifications:
• BLS
Notes:
• No weekends, no on-call required
• Mileage reimbursed (IRS rate)
• First-time travelers accepted
• Charting must be completed onsite (no take-home)
• Territory may include surrounding counties
