Description
Job Title: LCSW - Social Worker - Clinical
Location: Washington, DC 20037
Duration: 13 Weeks
Shift: 5x8-Hour Day (8:30 AM – 5:00 PM)
Hours: 40 hours per week
Requirements:
- Must live 50 miles away
- Minimum 1 year of hospital case management (discharge planning) experience required
- Knowledge of Cerner required
- Active LICSW and DC Social Worker license required
Job Responsibilities:
- Provide social work services to patients, families, and the community
- Coordinate and facilitate patient care during and after admission
- Handle discharge planning and care coordination efficiently
- Support patients through the continuum of care
- Adapt to changing work assignments as needed
