Description

Job Title: LCSW - Social Worker - Clinical
Location: Washington, DC 20037

Duration: 13 Weeks
Shift: 5x8-Hour Day (8:30 AM – 5:00 PM)
Hours: 40 hours per week

Requirements:

Must live 50 miles away
Minimum 1 year of hospital case management (discharge planning) experience required
Knowledge of Cerner required
Active LICSW and DC Social Worker license required

Job Responsibilities:

Provide social work services to patients, families, and the community
Coordinate and facilitate patient care during and after admission
Handle discharge planning and care coordination efficiently
Support patients through the continuum of care
Adapt to changing work assignments as needed
