Job Details:
📍 Job Title: RN – Home Health
📌 Facility Location: Milwaukee, WI 53214
⏰ Shifts: Day 4x10-Hour (08:00 - 18:00)
📆 Duration: 13 Weeks
📅 Schedule: Fri/Sat/Sun or Sat/Sun/Mon (approx. 12-hour days)

Job Description:

Home Health RN responsible for ~2 SOCs/day 2 additional visits
Productivity requirement: 30 points/week (must meet for guaranteed 40 hours)
Patient Population: Geriatric
Territory: Up to 60-mile radius from hub

Requirements:

Minimum 1 year recent Home Health experience
OASIS experience required
HCHB EMR experience required
Active RN license (in hand)
BLS certification required
First-time travelers not accepted

Additional Info:

No on-call, No holidays
Locals accepted at travel rate
Traveler must be flexible with territory
Charting must be completed by 9 AM next day
Timely communication required
