Job Description:
The Physical Therapist provides comprehensive rehabilitation services as part of an interdisciplinary care team. The role involves evaluating patients, developing individualized treatment plans, and administering physical therapy interventions based on physician referrals. The therapist is responsible for improving patient mobility, function, and overall recovery while ensuring accurate clinical documentation and adherence to care standards.

Responsibilities/Duties:

• Perform detailed patient evaluations based on individual needs
• Develop and implement age-appropriate treatment plans
• Provide physical therapy treatments to improve mobility and function
• Collaborate with interdisciplinary healthcare teams
• Document patient assessments, treatments, and progress accurately
• Adjust treatment plans based on patient response and clinical findings
• Respond effectively to changing assignments and patient needs
• Ensure compliance with hospital and regulatory standards

Requirements:

• Minimum 2 years of inpatient rehab or acute care experience required
• Degree in Physical Therapy required
• Active PT License required
• State licensure required
• BLS certification required
• First Aid certification preferred
• Primary source verification required
• Candidate's permanent address must be at least 100 miles away from the facility to be eligible.
