Description

Job Title: Home Health RN
Location: Alameda County / Contra Costa County, CA
Start Date: 06/15/2026
Duration: 13 weeks
Shift: Days, 5x8

Requirements:

Minimum 1 year Home Health experience
OASIS experience required
Active California RN License (in hand at submission)
BLS certification required
Must be comfortable working in a field-based Home Health setting

Preferred:

Experience with Kinnser/WellSky (training provided)
Strong home health documentation and patient management skills

Notes:

Weekend coverage as needed
Coverage area includes Alameda and Contra Costa Counties
