Description: This unit cares for patients that have been discharged from the medical center but are going to require 1 week of physical therapy/ speech therapy/ occupational therapy. The nurse will care for 1:5 patients on days, and 1:5-6 patients on nights. These patients require daily care activities. The patients can include post-surgical patients, post stroke care, chf, and any patient that needs prolonged physical therapy. Very team-based care. You will work with the physical therapy team and care management team to plan for the patients discharge upon admission. Patients occasionally will have IVs, require lab draws, but this is a non-tele unit.
