Requirements:

• Active Oregon Pharmacist License (or pending at submission)

• Minimum 1 year of Outpatient Infusion Pharmacist experience

• Experience with Oncology Infusion preferred

• Epic EMR experience required

• Community Hospital experience preferred

• Experience with IV compounding and infusion pharmacy operations

Certifications Preferred:

✅ Chemo/Hazardous Drug Certification

Clinical Responsibilities:

• Verify non-hazardous and hazardous outpatient infusion medication orders, including oncology therapies

• Evaluate and verify oncology and non-oncology treatment plans

• Review insurance coverage and authorization requirements prior to treatment initiation

• Perform IV infusion compounding product evaluation and verification

• Provide infusion medication education to patients

• Participate in infusion adverse reaction management and intervention

• Support medication dosing, therapeutic drug monitoring, and high-alert medication management

• Work collaboratively with outpatient infusion, Med Surg, ED, and Labor & Delivery departments
