Job Duties: See scope of work on pages 3 and 4.

Minimum requirements, including licenses and certifications: Candidate must possess a license and current registration as a registered nurse in New York State.

Schedule and anticipate length of assignment: OMH requires 2 candidates for day shift care, 2 candidates for evening shift care, and 2 for overnight care. Length of assignment is expected to be for at least a year but OMH cannot guarantee the assignment length.

Background check requirements: Fingerprint, SEL, SCR, SAM, Medicare, and Medicaid checks are all required.

Vaccination requirements: Measles, mumps, rubella, varicella, and influenza vaccines are required.

OMH required training (This training will be provided by OMH): New Employee Orientation and TRUST.

The maximum allowable hours per position for the term of any authorized user agreement resulting from this RFQ are: 4,160 regular hours, 96 overtime hours, and 128 holiday hours. The maximum allowable hours are not guaranteed.

Award(s) will be made based on a best value determination by the program. Factors that may be considered include but are not limited to: price, candidate resumes and/or interviews (when applicable), past vendor performance, and scheduling availability.
