Location: Home-based visits around Reading, Lowell, Lawrence and Salem area
Schedule: Monday–Friday | 9:00 AM – 5:00 PM
Assignment: 13 weeks to start (Temp-to-Perm &amp; Direct Placement options available)
Mileage Reimbursement: $0.76/mile After first visit to last visit.
Will primarily be doing intake, admission, and annual assessments and will typically do 5-6 visits/day (each one lasts 30-45 mins)
Experience in case management, home health, community health, AFC/GAFC, behavioral health, or social services needed.
