Required for Submission:

  1. Copies of all required certifications must be included.

  2. Complete all required Job ID submission questions.

  3. Two professional references from positions held within the past year are required.

    • At least one reference must be from a manager.

    • References cannot be pending.

    • Candidate must have no employment gaps greater than 30 days within the required work history.

  4. Submit a map showing directions from the candidate's permanent address (as listed on their driver's license) to the contracted facility.

  5. Provide the following education details for every degree earned:

    • Area of study.

    • Degree type (Associate, Bachelor's, Master's, Diploma, Certificate, etc.).

    • Institution city and state.

Submission Rules:

  1. Local candidates are not accepted.

    • Local radius is within 75 miles of the facility.

  2. FOS requires a complete 7-year work history (August 2015-Present), including:

    • Clinical and non-clinical employment.

    • PTO or medical leave.

    • Schooling or other periods not actively working.

  3. Epic experience is strongly preferred, especially for RN positions.

    • Candidates without Epic experience may be considered for difficult-to-fill positions.

  4. Candidates who cancel an assignment with fewer than 14 days' notice will be designated as Do Not Return (DNR) within the system.

  • Modules

    • Modules are completed during orientation and renewed annually.

    • Module requirements vary by specialty.

    • Module time is non-billable if completed within the first 8 hours of orientation.
