Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Conduct initial patient assessments and develop discharge plans, including arrangements for nursing homes, home health, and durable medical equipment.
• Coordinate with all payers, manage cases involving uninsured and homeless patients, and secure prior authorizations as needed.
• Manage a caseload ranging from 15-28 patients, providing efficient and comprehensive case management services.
• Utilize Cerner EMR for documentation and care coordination tasks.
• Work primarily in inpatient settings to plan and facilitate patient discharges.
• Ensure timely and effective discharge planning to optimize patient outcomes and resource utilization.

Qualifications

• Current and valid RN license with the ability to submit required documentation.
• Minimum of 3 years' nursing experience in an acute hospital setting.
• Prior care coordination experience in a CommonSpirit/Dignity/CHI hospital.
• Basic Life Support (BLS) Certification required.
• Strong customer service, interpersonal, and written communication skills.
• Ability to multi-task and work independently in a fast-paced and changing environment.
