Job Summary

Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

Coordinate patient care and discharge planning for acute care patients.
Collaborate with interdisciplinary teams to assess and meet patient needs.
Utilize Epic for accurate charting and patient tracking.
Ensure compliance with CMS and healthcare regulations.
Participate in weekend rotations and float between units as needed.
Conduct needs assessments and manage worker's compensation cases.

Qualifications

Current RN license in California.
Minimum of two years of acute care case management experience.
Proficiency with Epic charting system.
Experience with HMOs, IPAs, and advance directives.
Ability to float between similar units and work every other weekend.
Strong communication skills with interdisciplinary support staff.
