Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Perform care coordination and discharge planning for patients.
• Maintain compliance with hospital policies and procedures.
• Assess patients and participate in family meetings to develop treatment plans.
• Coordinate with social services for the needs of patients experiencing homelessness or mental health crises.
• Arrange durable medical equipment and transportation for patients independently.
• Work closely with interdisciplinary teams for optimal patient outcomes.

Qualifications

• Hold a current CA state RN license or be eligible upon start date.
• Minimum of 2 years of acute care hospital case manager experience required.
• Proficiency in computerized documentation and Epic charting system.
• Knowledge of care coordination, CMS guidelines, and InterQual criteria.
• Experience with discharge planning and utilization review in a hospital setting.
• Ability to manage patient care for adolescents, adults, and geriatrics.
