Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Manage case load of up to 20 patients to ensure coordinated and effective care.
• Utilize InterQual criteria for admission and discharge planning decisions.
• Perform care coordination and interdisciplinary collaboration with social services.
• Coordinate care for adult and geriatric patient groups effectively.
• Conduct needs assessments and facilitate discharge planning and DME orders.
• Provide ongoing support and communication for patient care needs with the team.

Qualifications

• Current California RN license required.
• Minimum of 2 years experience in acute hospital setting as a case manager.
• Familiarity with InterQual criteria and Epic charting system required.
• Ability to work at least 2 weekends in a 4-week schedule to meet department needs.
• Experience with HMO and IPA healthcare environments recommended.
• Strong interpersonal and organizational skills essential for effective care coordination.
