Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

Coordinate and manage patient care in alignment with facility and CMS standards.
Perform continued stay reviews and admission assessments.
Ensure compliance with HIPAA guidelines and facility protocols.
Plan and facilitate patient discharge and coordination of follow-up care.
Utilize Epic EMR for accurate and efficient charting.
Collaborate with interdisciplinary teams to optimize patient outcomes.

Qualifications

Active AK RN License required.
Minimum of 2 years' acute care hospital experience.
BLS Certification preferred.
Proficiency in Epic EMR is required.
ACM Certification preferred but not mandatory.
Experience in care coordination and utilization review.
