Job Summary
Cuready Healthcare Staffing is seeking a qualified RN - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Implement and manage patient care plans including care coordination and discharge planning.
- Organize and participate in patient and family meetings effectively.
- Conduct patient assessments and utilize review for treatment planning.
- Coordinate care with interdisciplinary teams and manage equipment arrangements.
- Manage patient information using the Epic charting system.
- Apply knowledge of healthcare regulations and policies in daily operations.
Qualifications
- Active and unrestricted RN license in California.
- At least 2 years of acute care case management and discharge planning experience.
- Experience with Epic charting system required.
- Strong understanding of healthcare regulations, including CMS and The Joint Commission standards.
- Capability to work every other weekend as needed by the department.
- Experienced with handling a daily caseload of up to 25 patients.
