Job Summary
Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate and manage patient care across the continuum of care.
- Conduct thorough assessments to determine necessary care plans.
- Participate in discharge planning to ensure patient safety and continuity of care.
- Work closely with interdisciplinary teams to coordinate and implement care strategies.
- Manage patient caseload effectively, ensuring best practices in patient care.
- Utilize Epic charting system for documentation and workload tracking.
Qualifications
- Active CA state RN license; acceptance of pending license upon start date.
- Minimum of 2 years of acute care case management/discharge planning experience.
- Previous travel experience required.
- Proficiency in Epic charting system required.
- Experience coordinating care and facilitating patient transitions from hospital to home or other care facilities.
- Ability to work weekends as needed to meet departmental requirements.
