References must be from assignments worked in the past 12 months. *Required certs need to be ACTIVE - nothing pending. NBRC Cert and BLS required, active and in hand at time of submission Locals accepted at same BR Background hits of any kind will be reviewed by the facility. Facility does not accept any license hits. Smoke Free Facility-MUST BE NICOTINE FREE- No Exceptions! -Negative 10 Panel + Nicotine (Cotinine) Drug screen required within 12 months of start. Must be updated annually. Permanent and per diem candidates cannot return to Cleveland Clinic hospitals for 1 year from their previous end date. IMPORTANT - BCI/FBI prints are time Sensitive - Must completed within 48 Hours of signing contract. Extensions: Candidates may extend with client for a period of 18 months; after which a 6 month break is required. Client may choose to defer the required 6 month break on a case by case basis.
