Job Description:
Supplemental Health Care is hiring experienced Home Health LPNs (1+ year in a Medicare-certified agency) for contracts in Yuma, Arizona, and nearby areas.
Qualifications:
- Active, unencumbered LPN/LVN license in the state of assignment
- Current BLS certification1+ year of recent experience (within 3 years) in home health, hospice, or skilled nursing
- Skilled in wound care, injections, med administration, and vitals under RN supervision
- Familiar with OASIS-E functional domains
- Clear background check, abuse registry, and driving record
- Health clearance with TB and required immunizations
- Reliable transportation and valid auto insurance
- Willing to travel within the assigned area and adapt to patient needs
Home Health Nurse Contract Details:
- $1,860 - $2,026 per week
- 13-week contract
- SHC Home Health Nurses are paid by the hour, not by the visit
- Visits typically last 30 to 60 minutes depending on the care plan.LPNs are expected to conduct 6–8 visits per day based on task complexity and travel requirements.Travel time is incorporated into productivity planning and may vary based on geographic spread.
Key Responsibilities:
- Provide skilled nursing services under the direction and supervision of an RN.Monitor patient status and immediately report changes to the supervising RN or physician.Accurately document all care provided in compliance with agency policies and state nurse practice acts.
