Job Description:
Supplemental Health Care is hiring compassionate and experienced Hospice RNs to provide patient-centered care for individuals and families in Gurnee, Illinois, and surrounding areas.
Qualifications:
- Active, unencumbered RN license in the state of assignment
- Current BLS certification (American Heart Association)
- CHPN certification preferred, not required
- 1+ year of recent experience (within last 3 years) in hospice, home health, palliative care, or acute care
- Strong knowledge of symptom management, pain control, and hospice documentation (CMS CoPs, Medicare Hospice Benefit)
- Clear background and driving record; current TB test and immunizations
- Reliable transportation and valid auto insurance
- Flexibility for travel, occasional evenings, weekends, and on-call shifts
Hospice RN Contract Details:
- $2,008 - $2,218 per week*
- Nurse will have a designated territory of coverage
- 4- 6 patient visits per day (45 - 90 minutes each, depending on need)
- Travel time included in scheduling
- All nurses on contract with SHC are paid weekly
Key Responsibilities:
- Perform comprehensive assessments and develop individualized hospice care plans
- Deliver skilled nursing care, symptom management, and emotional support
- Educate patients and families on hospice philosophy and comfort care practices
- Collaborate with the interdisciplinary team to review and adjust care plans
- Maintain accurate documentation in compliance with hospice standards
