Job Description:

Supplemental Health Care is hiring experienced Home Health LPNs (1+ year in a Medicare-certified agency) for contracts in Savannah, Georgia, and nearby areas.


Qualifications:

  • Active, unencumbered LPN/LVN license in the state of assignment
  • Current BLS certification
  • 1+ year of recent experience (within 3 years) in home health, hospice, or skilled nursing
  • Skilled in wound care, injections, med administration, and vitals under RN supervision
  • Familiar with OASIS-E functional domains
  • Clear background check, abuse registry, and driving record
  • Health clearance with TB and required immunizations
  • Reliable transportation and valid auto insurance
  • Willing to travel within the assigned area and adapt to patient needs

Home Health Nurse Contract Details:

  • $1,500 - $1,657 per week*
  • 13-week contract
  • SHC Home Health Nurses are paid by the hour, not by the visit
  • Visits typically last 30 to 60 minutes depending on the care plan.
  • LPNs are expected to conduct 6–8 visits per day based on task complexity and travel requirements.
  • Travel time is incorporated into productivity planning and may vary based on geographic spread.

Key Responsibilities:

  • Provide skilled nursing services under the direction and supervision of an RN.
  • Monitor patient status and immediately report changes to the supervising RN or physician.
  • Accurately document all care provided in compliance with agency policies and state nurse practice acts.
