Description
Good Samaritan Hospice is the only not-for-profit, community based hospice in the Roanoke and New River Valleys and SML. For 12 years straight, GSH has won gold in the Our Health Senior Living Awards category for hospice. We are seeking a RN Care Manager for our Roanoke location to bring comfort, compassion and quality care to patients and families when they need it most.As an RN Care Manager, your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention and family education. In addition to excellent written/verbal communication, problem solving, and decision making abilities, the candidate should demonstrate experience with developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team.
Working Conditions:
Primarily
works in the patient’s residence. Travel
to long-term care facilities, hospitals, and area agencies and businesses will
be necessary. Travel within the entire
Good Samaritan Hospice service area will be necessary. Overnight travel outside of GSH service area
may be necessary for continuing education.
Employee is at direct risk for exposure to infections and blood borne
pathogens.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
