Public Health Nurse II – Adult and Aging Division
The Department of Family Services (DFS) is seeking a Public Health Nurse II to support older adults, adults with disabilities, children, and caregivers through assessment, case management, care coordination, and Long-Term Services and Supports (LTSS) screening. This position conducts home and field visits, evaluates client health and safety needs, determines eligibility for Medicaid-funded LTSS programs, develops care plans, coordinates services, and provides health education and crisis intervention.
The Public Health Nurse II works collaboratively with multidisciplinary teams while maintaining a person-centered and culturally responsive approach. Responsibilities include managing a caseload, completing comprehensive assessments, making referrals to community resources, conducting outreach and health promotion activities, participating in trainings and meetings, and mentoring staff as needed.
Minimum Requirements
- Graduation from an accredited nursing program.
- Current licensure as a Registered Nurse (RN) in the Commonwealth of Virginia, or eligibility to obtain Virginia licensure prior to employment.
- Experience in public health nursing, case management, care coordination, long-term care, geriatrics, disability services, community health, or a related nursing field.
- Ability to conduct home and field visits and assess clients with complex health, functional, and social needs.
- Knowledge of nursing principles, health assessment, care planning, community resources, and applicable federal, state, and local regulations.
- Strong communication, critical thinking, documentation, and interpersonal skills.
- Valid driver's license and ability to travel throughout the community to conduct client visits.
Preferred Qualifications
- Experience completing LTSS screenings and Virginia Uniform Assessment Instrument (UAI) assessments.
- Experience working with older adults, adults with disabilities, and vulnerable populations.
- Knowledge of Medicaid-funded long-term services and supports programs and community-based resources.
- Bilingual language skills are a plus.
