Job Description
The Resident Nurse Care Manager (RNCM) is responsible for establishing a comprehensive care practice that entails patient assessments, care planning, implementation, coordination, and ongoing evaluation to ensure both medically and financially appropriate care is provided. Key responsibilities include:
- Facilitating interdisciplinary team care coordination.
- Managing the care of participants recently discharged from acute care centers and skilled nursing facilities.
- Collaborating closely with the interdisciplinary team, including Social Workers and Physicians, to collect essential medical and social information.
- Conducting assessments at participant residences when necessary, with mileage reimbursement provided for travel.
This position is primarily on-site, with occasional community visits required.
Program Overview
The role is part of a Program of All-Inclusive Care for the Elderly (PACE) focused on keeping older adults as healthy as possible while living in the community through clinics, home visits, and additional services.
Requirements
- Must be open to transitioning to a permanent position after the initial contract period.
- Experience with the geriatric population is highly preferred.
- First-time candidates are welcome to apply.
- Weekend availability is not required.
- No on-call duties are expected.
- Floating between units is not required.
- Local candidates are welcome, but relocation is not mandatory.
- Pending state license accepted.
