Position Title
Registered Nurse, ICU
Location
Gardner, MA
Shift Information
36-hour Day Shift: 7:00 am - 7:30 pm (Every 3rd weekend and holiday, Monday - Sunday schedule)
Essential Functions
- Conduct assessments (and reassessments), develop plans, implement interventions, and evaluate nursing care plans and patient goals.
- Provide patient care according to established standards of care, practice protocols, and organizational policies.
- Respect the individuality of each patient, ensuring care enhances their sense of dignity and positive self-regard.
- Deliver patient and family education based on identified learning needs, readiness to learn, and potential barriers.
- Collaborate in planning and preparing patients and families for discharge, utilizing support systems and resources as needed.
- Delegate and supervise nursing interventions performed by other staff appropriately.
- Prioritize patient care effectively, seeking assistance when necessary and recognizing emergencies for timely intervention.
- Document all relevant data, therapeutic interventions, and patient responses accurately and promptly according to standards.
- Provide thorough patient reports to healthcare team members to ensure continuity of care.
- Encourage professional development and learning among colleagues, participating in preceptorship and training opportunities.
- Identify risks to patient safety and seek opportunities to minimize medical or healthcare errors through appropriate reporting.
- Recognize ethical issues in patient care and develop plans to address them.
- Administer medications and intravenous therapy in accordance with physician orders and established guidelines.
- Assess patient pain levels upon admission and on an ongoing basis according to clinical indications.
- Reassess pain control effectiveness and notify physicians as required, documenting assessments and patient responses.
- Perform accurate identification of cardiac arrhythmias and interpretation of EKG configuration changes.
- Monitor and document thrombolytic and anticoagulation therapy per standards and protocols.
- Demonstrate knowledge of mechanical ventilation, including associated modes, settings, and adjuncts.
- Provide care for intubated patients, including suctioning and monitoring, while troubleshooting as needed.
- Accurately interpret arterial blood gases and assess features indicating impaired gas exchange and airway clearance.
- Understand hemodynamic monitoring processes, assist with equipment setup, and support arterial and central vein cannulation.
- Safely administer vasoactive medications, demonstrating knowledge of dosage, actions, side effects, and titration parameters.
- Conduct ongoing assessments and monitoring of patients in the immediate post-operative phase according to established protocols.
- Implement interventions for potential complications during the recovery phase of general, regional, or conscious sedation.
- Obtain necessary information for medication reconciliation documentation to support attending medical personnel.
- Implement components of the Ventilator-Associated Pneumonia (VAP) bundle consistently, alerting management of any anomalies for review.
- Actively participate in daily multi-disciplinary rounds for effective patient care planning and implementation.
Job Requirements
Minimum Education
- Graduate of an accredited school of nursing.
Minimum Experience
- 1 year of medical-surgical experience preferred.
- Previous telemetry experience required.
- ICU experience preferred.
Licensures and Certifications
- Current Massachusetts RN License required.
- ACLS certification upon hire or must obtain within 60 days if BLS certification is active.
- CCRN certification preferred.
Functional Demands
Physical Requirements:
- Exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently.
- Frequent reaching, handling, and feeling as part of patient care activities.
- Occasional stooping and kneeling required.
