Position Overview

We are seeking a dedicated Physical Therapist to join our outpatient team. The successful candidate will be responsible for planning and executing rehabilitation programs aimed at helping patients achieve a higher level of physical function. This role demands a strong understanding of various therapeutic techniques and a commitment to patient care.

Key Responsibilities

• Apply knowledge in anatomy, exercise physiology, biomechanics, kinesiology, neuroscience, and pharmacology to inform treatment plans.
• Collaborate with facility staff to ensure a clean, organized, and effective rehabilitation environment.
• Develop individualized Plans of Care based on initial evaluations and ongoing assessments, incorporating patient and caregiver involvement.
• Create appropriate home exercise programs and discharge instructions upon completion of the therapy plan.
• Document patient progress in accordance with approved medical documentation procedures.
• Evaluate patient mobility, strength, and functional capabilities while setting realistic goals for improvement.
• Implement and adjust treatment plans based on patient progress and clinical judgement.
• Maintain open communication with therapy staff, department personnel, and referral sources.
• Ensure patient records are compliant with third-party payer requirements.
• Stay informed of advancements in physical therapy through educational workshops and professional publications.
• Perform standardized physical therapy evaluations as per APTA standards and state regulations.
• Supervise physical therapy assistants, ensuring professional and ethical standards are upheld.

Qualifications

• Minimum of 1 year of experience in Physical Therapy.
• Experience in an outpatient setting.
• Active Physical Therapist license in Illinois.
• Current certification in Basic Life Support (BLS) from the American Heart Association.
