Job Description
We are seeking a skilled Transplant Coordinator with a strong background in hematology/oncology and experience in navigator or case management. The ideal candidate will coordinate patient care throughout the transplant process, from candidate identification through to post-transplant care.
Key Responsibilities
- Coordinate all patient care needs before, during, and after transplantation.
- Manage and track pre- and post-transplant testing schedules.
- Provide education to patients and families on lifestyle adjustments and medication adherence.
- Assure patient confidentiality and comply with HIPAA regulations.
- Collaborate with the transplant team for effective patient management.
- Monitor post-transplant patient progress and manage complications.
- Assist with organ offers, acceptance, and recipient preparation as needed.
- Conduct clinical assessments and interpret relevant diagnostic laboratory data.
Required Skills and Experience
- A minimum of 3 years of nursing experience, specifically in hematology/oncology.
- Experience as a navigator or case manager.
- Proficient in the use of Epic charting system.
- Experience in an acute hospital and outpatient clinic setting.
- Sound understanding of pre-transplant evaluations and assessments.
- Ability to monitor vital signs, fluid balance, graft function, and identify transplant complications.
- Strong organizational skills and the ability to work autonomously.
Preferred Skills and Certifications
- Certification in Basic Life Support (BLS).
- Experience with peripheral blood transplant and cellular therapy protocols.
- Certification in chemotherapy (ONS/ONCC) preferred.
Patient Care Details
- Care for adult and geriatric populations, particularly those with hematologic malignancies.
- Utilize proficiency in universal precautions and waitlist criteria for patient management.
Work Environment
This position involves coordinating care for multiple patients, ensuring compliance with regulatory standards, and participating in ongoing training and meetings as necessary. The role offers a structured schedule without weekend or on-call requirements.
