Assignment Details
Location: Nebraska
Contract Length: 12–13 weeks
Start Date: Approximately 2 weeks from offer
Schedule: Monday–Friday
Hours: 7:00 AM – 3:00 PM
Shift: Day shift
Mileage: Reimbursed at GSA rate (from first to last home visit)
Housing: Preference for housing near Kearney area
Responsibilities
Provide in-home physical therapy services to patients in their residences
Perform evaluations and develop individualized treatment plans
Treat patients to improve mobility, strength, balance, and functional independence
Educate patients and caregivers on exercises, safety, and home programs
Document evaluations, progress notes, and outcomes accurately
Coordinate care with interdisciplinary home health teams
Ensure compliance with home health regulations and clinical standards
Requirements
Active Physical Therapist (PT) license
Home Health experience required
Ability to work independently in a home-based environment
Valid driver’s license and reliable transportation
Must meet all compliance and credentialing requirements
Strong communication, organization, and clinical skills
