About the Job

We are seeking a skilled and compassionate Travel Physical Therapist (PT) to join a Skilled Nursing Facility (SNF) team. In this role, you will evaluate residents and deliver evidence-based rehabilitation services focused on restoring mobility, improving strength and balance, reducing fall risk, and maximizing functional independence.

Key Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans

• Deliver hands-on physical therapy interventions including gait training, balance work, strengthening, transfer training, and functional mobility

• Treat residents recovering from surgery, illness, injury, or chronic medical conditions

• Monitor patient progress and modify treatment plans as appropriate

• Provide patient and caregiver education on exercises, fall prevention, and safe mobility

• Maintain accurate, timely documentation in compliance with SNF regulations and facility policies

• Collaborate with the interdisciplinary care team including nursing, therapy staff, and physicians

• Support safe discharge planning and transition of care

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (MPT) from an accredited program

• Active PT license in the state of assignment

• SNF or post-acute experience preferred

• Strong clinical assessment, documentation, and teamwork skills

• Ability to adapt quickly to new clinical settings and patient populations
