About the Job
We are seeking a skilled and compassionate Travel Physical Therapist (PT) to join a Skilled Nursing Facility (SNF) team. In this role, you will evaluate residents and deliver evidence-based rehabilitation services focused on restoring mobility, improving strength and balance, reducing fall risk, and maximizing functional independence.
Key Responsibilities
Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver hands-on physical therapy interventions including gait training, balance work, strengthening, transfer training, and functional mobility
Treat residents recovering from surgery, illness, injury, or chronic medical conditions
Monitor patient progress and modify treatment plans as appropriate
Provide patient and caregiver education on exercises, fall prevention, and safe mobility
Maintain accurate, timely documentation in compliance with SNF regulations and facility policies
Collaborate with the interdisciplinary care team including nursing, therapy staff, and physicians
Support safe discharge planning and transition of care
Qualifications
Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (MPT) from an accredited program
Active PT license in the state of assignment
SNF or post-acute experience preferred
Strong clinical assessment, documentation, and teamwork skills
Ability to adapt quickly to new clinical settings and patient populations
