Job Description

Urgent Need

  • Required Licensure: Current RN Licensure in New York State
  • Required Experience:
    • Two (2) years of experience in a home setting within the last fifteen (15) years.
    • Recent experience conducting comprehensive assessments of patients.

Job Summary

  • Ability to review health status questions and make evaluations based on UAS criteria for enrollment.
  • Proficient in using technology in the workplace.
  • Knowledge of nursing principles, state laws, and regulations.
  • Excellent organizational, interpersonal, written, and verbal communication skills.
  • Ability to work effectively with multi-generational consumers and demonstrate sensitivity to their concerns.
  • Comfortable in a fast-paced, deadline-oriented environment and capable of managing multiple complex tasks independently.

Job Responsibilities

  1. Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences.
  2. Review consumer medical documentation and health referral forms.
  3. Enter evaluation data into electronic forms and transmit as required.
  4. Document concerns and issues that arise during evaluations.
  5. Emphasize continuity of care to reduce fragmentation and gaps in treatment.
  6. Discuss healthcare options and supports with consumers.
  7. Initiate communication for enrollment into managed care health plans.
  8. Enhance communication with interdisciplinary care team members for improved care coordination.
  9. Collect quality review data and document case notes.
  10. Identify opportunities for health promotion and illness prevention.
  11. Maintain knowledge of community resources and payor requirements.
  12. Meet performance standards and criteria for the position.
  13. Perform other duties as assigned by the Regional Nurse Manager or project management.

Additional Information

  • Candidates must be prepared to conduct hybrid tele and in-field assessments.
  • Required documentation includes resume, Nursys verification, NY License #, and BLS certification (if not already certified, BLS is required before the start date).
