Job Description
Urgent Need
- Required Licensure: Current RN Licensure in New York State
- Required Experience:
- Two (2) years of experience in a home setting within the last fifteen (15) years.
- Recent experience conducting comprehensive assessments of patients.
Job Summary
- Ability to review health status questions and make evaluations based on UAS criteria for enrollment.
- Proficient in using technology in the workplace.
- Knowledge of nursing principles, state laws, and regulations.
- Excellent organizational, interpersonal, written, and verbal communication skills.
- Ability to work effectively with multi-generational consumers and demonstrate sensitivity to their concerns.
- Comfortable in a fast-paced, deadline-oriented environment and capable of managing multiple complex tasks independently.
Job Responsibilities
- Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences.
- Review consumer medical documentation and health referral forms.
- Enter evaluation data into electronic forms and transmit as required.
- Document concerns and issues that arise during evaluations.
- Emphasize continuity of care to reduce fragmentation and gaps in treatment.
- Discuss healthcare options and supports with consumers.
- Initiate communication for enrollment into managed care health plans.
- Enhance communication with interdisciplinary care team members for improved care coordination.
- Collect quality review data and document case notes.
- Identify opportunities for health promotion and illness prevention.
- Maintain knowledge of community resources and payor requirements.
- Meet performance standards and criteria for the position.
- Perform other duties as assigned by the Regional Nurse Manager or project management.
Requirements/Qualifications
- Valid RN license in New York
- Minimum of one (1) year of Home Health experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
