Job Description

• Work days from 6:30 AM to 8:00 PM, but schedule may vary.
• Must have experience in Radiation Oncology or Oncology with brachytherapy.

Professional Credentials

• MGB Badge Photo: Recent color photo (within 6 months), plain background, no glasses.
• License/Certificate Verification: Primary source verification of nursing license (NURSYS and BON required). MA Conditional Licenses not accepted.
• MGB Criminal Background Check: Within 45 days prior to start, including international searches if applicable.
• MGB Massachusetts CORI: Criminal history check through Massachusetts CORI system.
• MGB Resume: Include past 7 years of work experience with city and state.
• MGB References: Two recent references, at least one supervisor or above in skillset.
• MGB Skills Checklist: Required for submission.
• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
• MGB Education Verification: Verification of highest education level achieved.
• MGB SSN Trace: Trace of addresses/aliases for past 7 years.
• MGB National Sex Offender Check: National registry check within 30 days of start and annually.
• MGB Exclusion Checks: Search of OIG, GSA, MassHealth, and NH Medicaid exclusion lists.

Personal Health History and Vaccinations

• Fit Test: N95 fit test within 365 days of start date, updated annually. No religious exemptions.
• MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
• Hepatitis B: Strongly preferred to have 3 vaccines or proof of immunity or declination.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions.
• TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date, updated if >6 month break.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date, updated if >6 month break.
• Tdap Vaccine: Preferred within 10 years and updated upon expiration.
• Respirator Medical Clearance (RMC) Attestation: Required if fit test is required.
• Positive PPD - Negative Chest X-Ray: If TB test positive, chest x-ray documentation required.
• MGB TB Symptom Survey: Required if positive PPD, updated annually.
