Qualifications

Required

• Licensure: Current RN Licensure in New York State
• Experience:

  • Two (2) years of experience in a home setting within the last ten (10) years, which may include:

    • Working in the field for a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC)
    • Conducting assessments similar to UAS, such as OASIS
    • Providing primary care to homebound persons
    • Care management for individuals receiving home and community-based services, including Hospice and Assisted Living
    • Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse caring for a minimum of 2 patients per week

Preferred

• Experience conducting assessments in UAS-NY (role 40 or 45) within the last 365 days
• Experience caring for a geriatric population and managing chronic diseases in a home setting
• Combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of 2 patients per week

Other Considerations

• Home care experience with only one patient (e.g., private duty nursing) does not satisfy required qualifications.
• Case management limited to utilization reviews or assessments conducted solely via telephone do not meet the required qualifications.

Job Summary

• Ability to review health status questions and make evaluations based on UAS criteria for enrollment.
• Knowledge of nursing principles, state laws, and regulations.
• Strong organizational, interpersonal, and communication skills.
• Ability to work effectively in a fast-paced environment and manage multiple tasks independently.

Job Responsibilities

1. Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences.
2. Review consumer medical documentation and health referral forms.
3. Enter evaluation data into electronic forms and transmit as required.
4. Document concerns and issues during the evaluation process.
5. Emphasize continuity of care to reduce fragmentation and gaps in treatment.
6. Discuss healthcare options and supports with consumers.
7. Initiate communication for enrollment into managed care health plans.
8. Enhance communication with interdisciplinary care team members.
9. Collect quality review data and document case notes.
10. Identify opportunities for health promotion and illness prevention.
11. Maintain knowledge of community resources and payor requirements.
12. Meet performance standards and criteria for the position.
13. Perform other duties as assigned by management.

• Candidates must be prepared to conduct hybrid Tele and in-field assessments.
• Required documentation includes resume, Nursys verification, NY License #, and BLS certification (if not already certified, BLS is required before the start date).
