Competencies

  • Medication Aide exam for CNAs in LTC Oregon Assisted Living facilities (if applicable)

Background and Compliance Requirements

  • Background Check within 45 days of start, covering all locations lived/worked in past 7 years
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
  • Education Verification required for all candidates
  • State-specific searches (Oregon Abuse/Neglect, Washington WSP/WATCH, OBRA Registry for CNAs in WA) as applicable
  • Drug Testing: 10 Panel Drug Screen within 90 days of start (no instant tests)
  • References: Minimum 2 supervisor references covering 1 year employment in specialty (6 months for CNAs)
  • Employment Application: Must include last 5 years of employment (7 years for APN/Subcontractors)
  • Skills Checklists: Annual updates required for applicable units

Health and Safety Requirements

  • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit Mask Test (annual, travel only)
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
  • Negative TB test or Chest X-Ray within 12 months, updated annually
  • MMR (Rubeola, Mumps, Rubella) vaccination or positive titer, or declination
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer, or declination
  • Seasonal Flu Vaccine during flu season or declination
  • COVID vaccine or declination
  • Respiratory Safety Clearance (annual for travel, patient-facing only)

Facility Specific Requirements

  • Float Policy Acknowledgement: Traveler may be required to float to other Providence facilities within reasonable distance
  • Badge Photo: Passport style with neutral background required
  • Epic Access Training: Completion of Epic modules required prior to or shortly after start
  • WorkRamp Modules: Completion of required Core, Specialty, Regional, and Annual Regulatory training modules via Aya's LMS platform
  • Additional facility-specific compliance and training modules as assigned

Schedule and Shift Details

  • Weekend Day Shifts: Friday, Saturday, Sunday
  • 12-hour shifts
